Is there a cancellation fee and/or no show fee?
If the cancellation occurs more than 48 hours from the appointment, no cancellation fee will occur. If a cancellation takes place within 48 hours of the scheduled time, a fee of $100.00 will be charged to the credit card on file. In the event of a no-show, a fee of $150 will be assessed to the credit card on file
What if I need to cancel or reschedule my appointment?
1 Balance requires 48 hours’ notice when cancelling or rescheduling. You may cancel or reschedule using the patient portal here.
Why do you require a credit card (CC) on file?
We require a CC on file for insurance co-pays and cancellation/no show fees.
Can you complete documentation for disability?
Yes, in most cases I will be happy to complete documentation for you. Care must be established for at least 3-6 months or 12 sessions (whichever comes first) in order to complete disability documentation.
